Upgrade TFS 2012 Update 4 to TFS 2015 Update 1 - New Server

I'm going to be upgrading our TFS 2012 Update 4 environment to TFS 2015 Update 1 and I'm hoping to get some help about the best approach.  This guide is a good start, but it doesn't get into some specifics I'm looking for.  Seems to be focused on doing the in-place upgrade.

Here's the current environment:

1 server.  Server 2008 R2.  TFS 2012 Update 4.  SQl Server 2008 R2.  

We've opted to stand-up a new environment and migrate over the data.

I know the new server will need to at least SQL 2012 SP 1.  I'd like to go to SQL 2014.

My questions:

On the new server, should I install TFS 2012 Update 4, bring over the databases into SQL 2014, and then perform an in-place upgrade to TFS 2015?  Or should SQL 2008 R2 be on the new server, databases restored, then upgraded to SQL 2014?  What order should this be done in given my goal to move over to a new server?

Is it ok to have TFS and SQL on the same server, or is this just asking for trouble and should it be seperated?  We are a small group.

Are there details about what volumes need to be created for SQL databases / logs and do there need to be special TFS folders for certain data?

I appreciate some guidance on this.
